 Welcome to North Colorado Health Alliance

 

The North Colorado Health Alliance will be a leader in improving access to health care services by:

 

  • Integrate the four main domains (physical, dental, mental, and drug/alcohol) of health care services accessed by un- and underinsured people in its service area.
  • Collect and analyze data about the population(s) served by its member organizations and use this data in planning new, expanded, or realigned services.
  • Improve the quality of care delivered by its member organizations through the use of automated point of care clinical guidelines, the application of best practices in the management of chronic diseases, the promotion of preventive care, and the monitoring of health status and outcomes.
  • Achieve economies of scale to assist its member organizations in implementing, sustaining, expanding or upgrading these practice automation systems.
  • Develop local funding source to support new and expanded services.
  • Maintain current inventory of Community Assets.
  • Maintain current data regarding population need and demand.
  • Stimulating innovation and integration.
